When to Keep your Child Home from School
Please note: Your child will be sent home from school if they have any of these symptoms for the health of your child and the health of all the other children in school. Also:
- Fever - 100.5° F or above (fever-free for a full 24 hours (without taking a fever-reducing medication) before returning to school.
- Colds/Flu - Severe chest congestion or a significate cough
- Pink Eye (Conjunctivitis) red, painful, itchy eyes, especially with discharge or crusting, should have a medical evaluation by a physician. Conjunctivitis Information
- Fatigue and/or tired from an illness
- Severe ear pain - evaluation by a physician
- Significant nasal draining
- Vomiting/Diarrhea
- Fungal infection
- Strep Throat - (accompanied by a fever or feeling ill) Strep Throat Information
- Head Lice - If your child has a known or suspected case of head lice, please notify the school nurse. If your child has active head lice, it is recommended that they be treated with a medicated shampoo or lotion.
- Rash - Students with a questionable rash should be referred to a physician for a medical diagnosis and to determine if the rash is caused by an infectious disease.

Health Examination (Physical):
Education Law Article 19 and Regulation of the Commissioner of Education requires health examination for all students who are:
- Entering the school district for the first time
- In UPK, Kindergarten, 1st, 3rd, 5th, 7th, 9th and 11th grades
- Participate in strenuous physical activity, such as interscholastic athletics
- Requesting an employment certificate
Children should have a physical done every year around their birthday. ALWAYS keep a copy of your child's physicals in a safe place at home. Do not assume your doctor's office will fax the school a copy. Please ensure Caledonia-Mumford Central School receives a copy of your child's physical. You can confirm a copy has been received by calling the school nurse at (585) 538-3409 (Elementary) or (585) 538-3454 (MS/HS) .
Dental Health Assessment:
Dental certificates stating that your child has been seen by a dentist or dental hygienist are also requested at the same time as grade-level physical requirements.
Hearing and Vision Screening:
Commissioner's Regulation §136.3(e)(1)(iii) requires hearing screening and vision screening to be conducted in school if results are not provided to the school on the student's health certificate (physical exam completed by a private health care provider) within six months of admission to the school and in grades Pre-K or Kindergarten, 1.3.5.7 and 11, and at any other time deemed necessary. The hearing screening shall include, but is not limited to, pure tone screening. The vision screening shall include but is not limited to, distance acuity, near vision, and color perception.
Scoliosis Screening is also conducted at school for the following:
- Girls 5th and 7th grade
- Boys 9th grade
Immunizations:
In accordance with Public Health Law #2164, all children entering school must be immunized against polio, measles, rubella, mumps, hepatitis B, diphtheria, and varicella; meningococcal (1 dose grades 7,8,9,10,11; booster at age 16), for preschool add Haemophilus Influenzae type B (Hib) and pneumococcal (PCV).
At the time of registration proof must be presented that these immunizations have been completed or are in the process of being completed. If booster shots haven't been given, please let the school nurse know the scheduled date.
NYS Immunization Requirements for School:
Medication Policy:
Per NYS Law, School Nurses cannot administer medications to students without written authorization from their primary car physician, including prescription and over-the-count medications.

An adult must drop off the medication. Your child is not allowed to carry medication to and from school.

Medication must be in the original container with your child's name on it.

Over the counter medication must be in an unopened bottle.

A new medication authorization/permission form (doctor's orders) must be filled out each year by your physician.

Parent /Guardian must fill out the medication authorization form and submit it to the school nurse with the medication.

New medication authorization forms are sent to the health office before the 1st day of school. This will ensure your child has the medication they need on the 1st day of school.
